Project: Discuss the Decreasing Incidences of Hypertension in African American Adults in the Primary Care Setting through Education and Nutrition

1) Identify the translational framework used during  project planning, implementation, and evaluation

2) Be sure to explain how this framework was used for  DNP Final Project.
