Assessment Specification – what you need to do:
Provide a full exploration of the issue previously debated in the formative assessment (Part A)
⦁ Identify the key elements that support the implementation of the aspect of policy (self-care) and explore the relevance for patients, carers and practitioners.
What is meant by key elements? think of key elements as approaches, interventions, initiatives, enablers or things that can support people to manage their condition, that can support them to self-care, things that will give them choice that will give them some control, that will enable them to have a say in the management in their condition, empower the patient to take control of the management of their condition.
Key element for example ‘personal health budgets’ select two or three key elements.

⦁ Discuss the advantages and disadvantages of implementing the elements in practice for both the patient and the practitioner.
Considering the patient, carers and practitioners. Considering how patient centred care, how important that is and how important collaborative working is.

⦁ Explore the contribution of nurses, health professionals and other agents that form the collaborative team in providing integrated person-centred care
