Assessment of family health risks requires many approaches. This assignment will focus on two tools, the genogram and ecomap, to help identify factors that contribute to a family’s health and the achievement of healthy outcomes. There are five elements to this project.

Element 1: Develop your family genogram.

Go to the CDC’s My Family Health Portrait to develop your genogram.
Follow instructions on the website.
Print your final genogram and table as a PDF or to a Word doc.

Element 2: Develop your family ecomap using any of these three methods.

Download a free ecomap template from the internet.
Create an ecomap using computer software.
Draw an ecomap on paper and scan it to a PDF.

Elements 3-5: Analyze your findings in a formal APA paper using the following headings

Discuss how nurses use genogram and eco-map tools.
Significant Findings
Identify the significant findings you identified for your family.
How to Incorporate These Tools in Nursing Practice
Discuss how and why a nurse would incorporate a genogram and ecomap into the care of your family.
