Dear Ms. Sosa:
‘Ibis &lice has received information indicating that you may have violated the Nursing Practice Act and/or the Board’s Rules and Regulations Relating to Nurse Education, Licensure and Practice. The Board is required, unless it would jeopardize its investigation, to provide notice that a complaint has been filed and to give notice of the complaint. We are initiating an investigation and providing you with an opportunity to be heard and to submit information in writing about each of the following alleged facts or conduct to assist us in making a determination:
1. On or about May 23, 2022, and May 26, 2022, while employed as a Registered Nurse with Kindred at. Home Hospice,. San Antonio, you falsely documented home visits for Patient M.C. who was ordered to have home visits twice a week.
In addition to your response regarding the above allegarionin you arc advised to:
• Provide documentation recording mitigating circumstances as applicable. Refer to the included listing of various mitigating circumstances and factors to be considered from Board Rule 213.33(b)&(c):
• Provide any other information showing compliance with the Nursing Practice Ace and the Board’s Rules and Regulations Relating to Nurse Education, 1.kensuce cor \\censure retention, including, for example, recent positive employment evaluations, successful completion of subsequent remedial education or other continuing, nurse eduent:tort,andlor certification/s earned;
• Provide an up-to-date resume or work history;
• Notify us of any changes of position or employment while these matters are pending, Notify us of anyplans to enter or a to nursing school for a subsequent degree.,
• Complete and return the enclosed questionnaire; and • Notify writing within ten (10) days of any change of name or address.
