Describe a standardized method to identify and prioritize health needs within and across “communities of concern” for the purpose of federally mandated Community Health Needs Assessment (CHNA).

The method integrates qualitative and quantitative data, offering a best practice strategy for conducting CHNAs using publicly available data (such as census tract or zip code) and local stakeholder input. The article is based on work done in partnership with 6 hospital CHNAs in Northern California.

1. What does CHNA stand for?
2. What is the purpose of a CHNA?
3. Identify the four steps identified in the article to identify prioritized significant health needs? (Figure 1)
4. Identify the four components of identifying the geographic areas in a hospital service area to identify Communities of Concern.
5. What were the quantitative data indicators used to determine Health Needs: health outcomes and health factors?
6. Why do you think it is important to first look at quantitative data from secondary sources but also consider qualitative data collected from key informants, community health experts, and community members?
