Reflecting on your clinical experience thus far and the EBP approaches you have learned in class, choose one thing you would like to see changed in your clinical site: a patient outcome, flow issue, communication issue etc. Read through the Institute for Healthcare Improvement (IHI) instructions to create a Cause and Effect Analysis (AKA Fishbone diagram) which is posted on Blackboard. This also includes a fillable PDF for use to create the Fishbone diagram.

⦁ Write the effect you would like to influence in the box on the right side of the paper (10 pts)
⦁ In each of the 5 categories generate a list of causes and sub causes (see example posted on Blackboard) until each cause is specific enough to test a change and measure its effects.  (60 pts)
⦁ Identify where you think a test of change should start and why (30 pts)

Meets expectations: 83-89
Partially meets expectations: 80-82
Does not meet expectations: ≤ 79
