What is a data dictionary and how is it used in healthcare?

What are the benefits of using a data dictionary? Describe the purpose of metadata in a data dictionary and provide examples.

Select one of the scenarios below and create a data dictionary for that situation (an example is included below). You will need to use your imagination to come up with the necessary details, such as what type of data is being collected! Provide a brief description of your scenario along with your data dictionary. At minimum, you need to have at least 4-5 fields, and cover the data type, definition, format, field size, value range, and whether or not it is a required field. Feel free to include more!
