Write a case study of centre on a patient who each student has been involved in the care of within their area of practice. The patient should be selected based on palliative care role in diagnostic reasoning, medications management and subsequent therapeutic interventions for that patient. The case study must not exceed 500 words in length. However, figures, tables and boxes can be used and will not be included in the word count. The case study should be presented at the beginning of the essay.

Formulating your case study based on the learning outcome

• Background/biography of your patient (this should include a health history)

• Current health status of your patient: current suspected medical diagnosis (i.e. the reason for the need for current healthcare); current suspected nursing diagnoses (needs for nursing within activities of daily living or other framework); detail of signs, symptoms, investigations and so on (use tables, figures etc.)

• Consultation process

• Confirm medical and nursing diagnoses

• Medications prescribed

• Shared/collaborative care

• Brief overview of subsequent care journey
