To prepare:
(a) Conduct a Comprehensive Psychiatric Evaluation on a patient with Post-Traumatic Stress Disorder using the attached.
(b) There is also a complete exemplar document that you can use to see an exemplar of the types of information a completed evaluation document should contain.
(c) Develop a case presentation, based on your progress note of the patient, that includes chief complaints, history of present illness, any pertinent past psychiatric, substance use, medical, social, family history, most recent mental status exam, and current psychiatric diagnosis, including Differential that was ruled out.
(d) Include at least five scholarly resources to support your assessment and diagnostic reasoning.
ASSIGNMENT:

(a) SUBJECTIVE: What details did the patient provide regarding their personal and medical history?
(b)What are the symptoms of concerns?
(c) How long have they been experiencing them?
(d) What is the severity?

ASSESSMENT: What were your differential diagnoses? Provide a minimum of three (3) possible diagnoses, and why?
(E) REFLECTION NOTES: ((1) What would you do differently in a similar patient evaluation? Reflect on one social determinant of health according to health people 2030 ( you will need to research) as applied to this case in the realm of psychiatric and mental health. As a future nurse, what are one health promotion activity and one patient educational considerations for the patient for improving health disparities in the realms of psychiatric and mental health? Demonstrate your critical thinking.
